Should be close up shot, passport size, colored with white background, no accessories, left and right ears visible, handwritten name tag with signature - Photocopy and original Valid ID
It can be SSS ID, GSIS ID, Philhealth ID, Passport, Postal ID, License or a valid ID that contains your information
Qualifications:
- Must be 18 years Old
- Filipino Citizen
- No criminal Record
- No previous work at a government or military service and has dishonorable background and got fired
- Did not take the same level exam within the last three months
