Person With Disability ID Application

PWD ID is an identification card for a person with a disability in the Philippines. If you have this kind of ID you will be able to take advantage of all the PWD benefits that were included and mandated by the law. If you don’t have an ID yet, here’s a PWD ID application guide.

Person Who Is Eligible To Get PWD ID:
-Hearing Disability
-Speech Impairment
-Multiple Disabilities
-Mental Disability
-Learning / Intellectual Disability
-Visual Disability (Impairment that can not be corrected)
-Orthopedic Disability
– Psychosocial disability (Depression, Mental and Behavioral Disorder)
– Disability resulting from a chronic illness  (Cancer, diabetes)

Requirements To Get PWD ID:
– Completed PWD ID application form
-1×1 ID photo with signature at the back (2 pcs)
-Barangay Clearance
-Any valid government-issued ID
-Latest medical documents or medical certificate

PWD Application
Attach your ID pictures and medical certificate or documents to the accomplished application form. Prepare all the requirements and once all the documents were completed bring them to any of these following government offices:
– Persons with Disability Affairs Office
-Office of the Mayor
-Office of the Barangay Captain
-City/Municipal Social Welfare and Development Office
-City or Municipal Health Office or its satellite offices

Don’t delay your application. Get your ID as soon as possible so that you can take advantage of all the benefits. Remember that if it is your first time getting a PWD ID you are free of charge and the ID validity is up to 3 years.
