How To Be A PhilHealth Member Voluntarily

Persons Who Can Apply:

  • Informal sector workers
  • Self-employed professionals
  • Overseas Filipino Workers
  • Dual citizens
  • Naturalized Filipino citizens 
  • Expats 
  • Any Filipino who is not 60 years old and above, does not have a fixed employment term and should not be an indigent member

Afterward, you need to submit PhilHealth Member Registration Form (PMRF) if you’re applying to be a member voluntarily.

Voluntary Online Application Process:

-Visit https://eregister.philhealth.gov.ph/ and click proceed
-Read terms and condition and once done, click the “Accept” button
-Provide all the required information
-Upload your document
-Enter the Captcha code, click the box and click “Submit Registration”
-Once registration has been submitted, an instruction will be sent to your email on how to complete your application
-Make sure to copy the transaction number because that will serve as your reference

Voluntary Application Process in a Philhealth Office:

-Visit your nearest Philhealth office or outlet
-Fill out two (2) PhilHealth Member Registration Form
-Submit the accomplished form to Philhealth to their employees
-Once verified, wait for your Member Data Record and ID to be processed and issued
-Once the ID card has been issued. Use your Philhealth ID number to pay for your premium contribution

Being prepared is very important especially when it comes to a medical emergency.
