1. Must provide Certificate of Live Birth (4 copies)
2. Affidavit for Delayed Registration at the back of Certificate of Live Birth by the father, mother, or guardian, declaring therein, among other things, the following:
-Name
-Date of birth
-Place of birth
-Name of the Father (If illegitimate)
-Date, Place of Marriage and Reason for not registering within 30 days after birth (If legitimate)
*If ever the person attempting to register the birth of an illegitimate child is not the mother, the person attempting to register the child must declare in a sworn statement the recent whereabouts of the mother

3. Documentary evidence that will show the name, birthday, birthplace of the child, name of the mother and father such as:
-School records
-Baptismal certificate
-Medical records
-Income tax return of parents
-Insurance policy
-Barangay Captain’s Certification

4. Affidavit of two unrelated persons who might have witnessed or known the birth of the child
5. For 18 years old and above, a Certificate of Marriage is needed if married

Here’s How to File a Late Registration:

• Visit the Office of the Civil Registrar in the municipal or city hall of the place where the birth occurred
• Submit all the requirements needed
• Once all the requirements needed was already submitted, the registrar will check the Certificate of Live Birth presented if it is completely filled with all the requirements needed
• If a person who filed for late registration is a foreigner, he/she must submit a travel document that shows the origin and nationality of the parents
