Having a Government-issued ID is very important to everyone, especially if you’re going to transact with public and private institutions. It is very vital in our everyday life, but what if you lost or damage your ID? What will you do? No worries, here’s a guide on how to replace lost Valid ID.

Voter’s ID Replacement

Requirements needed:
-Request form
-Affidavit of loss

Here’s the process:
1. Proceed to the local COMELEC office
2. Fill-up Request Form for Replacement of Voter’s ID
3. Once you’re done, attach your affidavit of loss to the request form
4. Submit it to the COMELEC employee
5. They will give you a claim stub with date on when you’re going to pick up your ID

Driver’s License Replacement

Requirements needed:
-Application Form for Driver’s License
-Proof of Identity
-Affidavit of Loss

Here’s the process:
1. Proceed to Land Transportation Office
2. Go to the Customer Service Counter and get a Driver’s License Application Form. Get a queue number and wait for your number to be called.
3. Once called, proceed to the evaluator and submit all your requirements
4. Proceed to the Photo/Signature taking area
5. Once done, pay for the fees. Do not forget your receipt
6. Proceed to the releasing counter, provide the official receipt and claim your card
7. Sign the appropriate Released Form

SSS ID Replacement

Requirements:
-2 valid IDs
-SSS Form E-6 (4 copies)
-SSS Form R6
-Php 200, replacement Fee
-Affidavit of Loss

Here’s the process:
1. Fill-up Social Security ID E-6 form
2. Get a queue number and wait for your number to be called
3. Once called, go to the counter and submit all the required documents
4. Once done, go to the Payment window and pay for the replacement fee
5. Proceed to the picture and signature taking room
6. Wait for your ID to be delivered at your preferred delivery address within 2-3 months

PhilHealth Card Replacement

Requirements:
-Valid IDs to prove your identity

Here’s the process:
1. Visit PhilHealth Regional Office
2. Present your ID to prove your identity
3. Once done, wait for the officer to process your application
4. Wait for your ID to be processed
5. Once done processing, get your ID and you’re good to go
