• Choose which certificate you wanted to get, either:

• Afterward, you need to choose whose certificate you’re requesting for, either:
  • Self
  • Son
  • Daughter
  • Father
  • Mother
• Fill up all the required information
• Your name will be displayed again for you to double-check, if no correction is needed click “Continue”
• Enter your Father’s full name and mother’s maiden name
• Select your Place of Birth
• Choose the purpose of your request and if there are any legal proceedings done with the certificate
• Fill up the delivery and Contact Information
• Afterward, the summary of your certificate will be displayed. Double-check every detail
• Then lastly, pay for the fee and wait for it to be delivered
